Student/Resident Rotations at Martinsburg

At least four weeks before your desired rotation start date

Once your rotation request has been approved and your preceptor has been assigned you will need to fill out the orientation packet. Use the 'Student Orientation Checklist' to help keep you organized. The forms below will all need to be filled and returned to the Affiliation Coordinators within this time frame.

Random Drug Testing Notification and Acknowledgement

All trainees must sign and submit an influenza vaccination form to indicate whether you have been vaccinated, provide necessary documentation if you have been vaccinated elsewhere and acknowledge that you have read, understood, and agree to comply with the requirement that in the absence of vaccination you will be expected to wear a face mask throughout the influenza season (Dec. 1 through March 31). The date may be extended based on local influenza activity.

You will also have to supply:
  • PPD, current less than 1 year old or chest X-ray done within the past six months
  • Hepatitis B Vaccination or waiver
  • Copy of BCLS certification
  • Copy of ACLS certification for Surgical rotations
  • Certification of training in the Talent Management Systems (TMS):


In addition to the above, there is also material that will have to be read and reviewed this will be sent to you in a Virtual Orientation Packet via email after you have sent in your paperwork.

At least two weeks before your desired rotation start date

Photo identification is required to be worn at all times and be with you when you are entering and leaving the facility.

It is during this span of time that you must schedule an appointment with the Personal Identity Verification (PIV) office located in Building 217 at the Martinsburg VA Medical Center (VAMC) campus, 304-263-0811 ext. 2066 or ext. 2067 to get your fingerprints. Acceptable Forms of ID for PIV Card


Fingerprints can be taken at any Department of Veterans Affairs VAMC as long as you call to schedule an appointment. Locations and phone numbers can be found at locations. It is very important to tell them that the fingerprint results need to be sent to the Martinsburg VAMC and provide them with these two numbers:

  1. SOI VAB9
  2. SON 1316

If you fail to provide them with these two designating numbers the results will not be sent to the Martinsburg VAMC. This will result in a delay or cancellation of your rotation/training.

Mini Orientation Monday - First day of your rotation 

8 a.m.: Education Department Building 207B classroom

If a Monday Mini-Orientation falls on a federal holiday, orientation will be moved to Tuesday
