Primary Care Service oversees the delivery of Primary and Emergency Department care in the implementation of The Department of Veterans Affairs patient-centered medical home model, the Patient Aligned Care Team (PACT).
Primary Care Service honors America’s Veterans by providing quality and accessible primary care to all Veterans through PACT, placing the Veteran at the center of their health care team. The PACT model allows Veterans to receive pharmacotherapy, social work and nutrition care in their primary care setting.
Primary Care Service promotes team based, Patient-Centered Care (PCC) that focuses on a personalized, integrated, comprehensive and coordinated approach to health care. Primary Care and Mental Health Services partner to provide collaborative and integrated services through Primary Care-Mental Health Integration (PC-MHI) allowing Veterans to receive many Mental Health services within the primary care setting.
The Martinsburg VA Medical Center offers primary care at the main facility as well as seven Community Based Outpatient Clinics (CBOC). All CBOCs offer Women’s Health, pharmotherapeutic and anticoagulation management, as well as a variety of specialty care services that are available at certain CBOC locations, including, physical therapy, occupational therapy, tele-psychiatry, tele-retinal imaging, tele-dermatology, tele-podiatry, and more.
